Please supply the contact details of 2 domestic customers for whom you have installed a Gravelrings or Grassrings system (of at least 30m²) during the past 18 months who may be contacted as referees. These installations must have been secured and completed under the name of the company/business applying for approved installer accreditation. Where I/we provide you with personal data (“data”), I/we understand that the data will be held securely in confidence and processed for the purpose of carrying out your manufacturing and supplies of construction materials business and associated activities (“Activities”). In considering my/our application I/we accept that you may consult with and disclose the data to credit referencing agencies, banks, credit insurers and other responsible organisations outside your business that you have nominated (“third parties”), and that such third parties may process the data. We may also make enquiries about the principal directors with a credit reference agency. I/we understand that under the data protection act I/we have a right to know what data you hold on me/us if I/we apply to you in writing and pay the applicable fee.

I confirm that my company/business has, and will continue to have, the necessary insurance cover as described in section 1 of the approved Installer application form and I will supply evidence of cover on request. I understand that continued membership of the approved Installer programme is subject to adherence to the Beauxfort approved Installer application process, ongoing monitoring/inspection and adherence to published Beauxfort installation guidance and specific advice given by Beauxfort personnel. I understand that if my company/business does not adhere to these conditions, Beauxfort reserve the right to remove my company/business from the approved installer programme and its associated benefits.
